Please note: This is not a uniform application for all of the scholarships listed in our Resource Guide. Consult the individual scholarship descriptions to determine whether a separate application is required.

A PDF of the Scholarship Application is available on-line. The file name is PMYFEEFapp.pdf. Search for this name on your hard disk if you cannot find the PDF file after downloading it.

You need the free Adobe Acrobat Reader or Acrobat v 5.0 or greater installed to use this file.

With Acrobat Reader installed you may fill out and print a copy of the application. You must fill out the whole form or page(s) of the form in one session, unless you have a full-version of Acrobat which will allow you to save your work. Note: Using the free version of Acrobat Reader you cannot save your work.
A strategy for working with the form: Print out a blank form using Acrobat Reader, Fill out the blank form completely by hand, come back to Acrobat Reader to fill out the electronic form, print the filled out form, sign and send in the completed form.

If you have problems printing the Scholarship Application, please request one from the PMYF office.

Please do not use previous years' applications, as questions may change from time to time. Please submit the application in the format provided (that is, do not re-type it, scan and re-format it, etc.).
